I wrote a guide on this months ago but it seems to have been deleted... so here it is again from scratch (some info may be wrong and I appreciate any input)

Spearblock - DR 5/Slash or Bludgeon

Bramble-Casters - Gloves: Greater Spearblock (DR 10/Slash or Bludgeon)
Granite Plate - Half Plate: DR 2/-
Bloodrage Symbiont - Trinket: DR 5/-
Ironweave Tunic - DR 3/Bludgeoning
Titanic Docent - Titan Shield (DR 30/- and SR 30 for 15 seconds. 1 charge/rest)

WF Dmg Reduction (DR 1/2 Adamantine for I/II at lvls 5/10, adds to Adamantine Body)
Barbarian Imp DR (DR 1/2 at lvls 4/10, adds to Barbarian DR)

Stackbility:
ONLY 1 SOURCE OF DR (Item or Racial or Spell or Boost, the highest) WILL STACK W/ BLOCKING DR.
i.e. going Warforged Barbarian is pointless in this respect (WF DR does not stack with Barbarian DR)

Max Achievable DR:

(Blocking DR)
12/2+2 = 8 (BAB/2+2)
4 (Tower)
5 (+5)
6 (SM and iSM)
6 (Dwarven SM 3)

(One additional source of DR)
5/10 (Item or Stoneskin) | 6 (Lvl 11 Barb w/ Imp DR 2) | Barb AB V (15 for 20secs)
