Yes contact customer service.

I believe only the leader may.

Costs 14.95USD (IIRC) and don't expect to get the name you want since there are literally thousands of dead guilds in the system.

It seems once a guild has been created - it (the name) is from then on reserved and unavailable.

So if you still want to change a guild name - expect a LONG wait - from 2-6 weeks; and have a LONG list of names (prioritized and tell them that) ready.

Sorry the information I provided is out of date - it is only a month or two old - we went through all that with a name change recently.
