Unable to report multiple bugs in bug reporting tool without moving away.
After submitting a bug and having it confirmed, the Report a Bug button does not allow you to report an additional bug, you need to select a different button then go back to it. Which is just untidy.

Ice storm (and Glitterdust) can persist through death. It then becomes permanent.

Slavers of the Shrieking Mines - optional to kill 2 Beholders cannot be completed. Not new, it's been like that for as long as I can remember.

Mouse-over text on the map for all seven quest givers in House Jorasco. I first noticed this a few weeks ago. Since then, it has happened every time I checked. I have not noticed this anywhere else, but I didn't run around checking, either. Two screen shots are included for illustration, to include the other five seemed excessive. (The two raids and Derek Grospic display correctly.)
