I usually keep the hirelings on aggressive, so there could be something to this. I'll have to be more careful with them and see if that helps on the ones that just won't activate.

Ok, it probably has been in place for years and I've just been playing too many casters recently. It still is odd that some abilities prevent false attacks while others don't. It seems like you have a way to stop that behavior coded in, so why aren't all active attacks working that way?

I'm not sure how to explain it differently, but it definitely isn't lag, so I'll try again since most people are latching onto that.
I click Hand of Harm (for example) once and it successfully works. I can then keep hitting the button while it is on timer and I will keep performing attack...
This isn't a lag issue, I can swing full, pointless attack chains by continuing to hit just Hand of Harm over the 9 second cool down. If I try the same on trip or sunder, I just do the first attack that works and then nothing else happens while mashing that attack while it is on cool down.
Has it always been the case that activating an ability that is on timer (such as a special melee attack like the new monk katas) that you will swing your melee weapon and it will not actually make an attack, thus wasting the animation time? Trying to activate trip or sunder just doesn't do...
Add one more vote for the GMoF core 4 to allow for balance between light and dark and give access to the other path on top of the dodge bypass and MRR cap increase. Maybe also increase the base absorption from the enhancements in the tree by 10%.

With all the complaints about meditation, you could put a replacement for Lighting the Candle where you can still function after hitting meditation. Meditation really doesn't have a place in normal gameplay where we are moving around completing the quest.
