It still may be an issue of the server not evenly distributing people across instances. According to Cordovan, the main reason they brought the server down on Wednesday was because it was putting too many people in the same instance, as he put it, too many people in too small a space, so I wonder if it's still not distributing people as well as it could be doing.
One area I have noticed lag improving, at least in the 32-bit side of things, was The Shroud. I've had an opportunity to run it a couple times since they worked on it, and I was anticipating the crippling lag after each section going to the chests but it never happened until the final chests. I have yet to run it on Cormyr, but as Tolero has said the quests are the same across both 64 and 32 bit architectures, so I'm looking forward to seeing if it runs the same.
Also on the last Friday's at Four, Cordovan addressed the fact that the DDO team knows about hitching issues and it's something they're working on. I've noticed it across both architectures, maybe more so 32-bit, but at least they are aware of it and are working on it.
