Crucible - Bug, or general bug?

Had an odd occurance the other day while finishing crucible. Made the swim for the test of endurance, Popped up to get the horn. Normal mobs with 3 Reapers. Between being Plagued, feared, acid clouded and ice stormed I had a HP Failure.

Jibbered, finished them off grabbed the horn and beat feat back. I got pretty much to all the way to the locked door, jibbers expired horn fell to the ground.

Funny part is, the Horn was then bugged and could not be picked up.

Is this related to being jibbered and dieing? Has Anyone had a similar experience with a quest item dropping while jibbers wears off and then not being able to pick it up?

I know crests have fallen thru the ground before in quests; so doesn't surprise me that quest item could fall in such a way as to not being able to be picked up. Probably has nothing to do with being jibbed (other than it being the cause of death while holding the quest item) and entirely due to dying & the quest item glitching into the terrain on your death.

Nasty bug, although as Rabidfox says it is probably due to a glitch in the terrain. I am terrified of dying in The Underlane Assignment because very often the crests remain under the ground and cannot be picked up. Only solution, restart the quest.
