I personally haven't seen or felt it be any different since the update. I ran the new quest on R1 with a few first lifers in the group and a cleric that didn't heal whatsoever. They all died multiple times so maybe for whales its easier but not the general public.
Did you check whether that cleric is a Dark Apostate archetype?
Usually, Dark Apostate players when they play that archetype, they do not intend to heal other people other than themselves, also, they have to be undead to get healing.
I think it's a failure for the developers to provide
fun for the casting classes like Favored Soul and regular domain-based clerics. Spells like divine wrath, reborn in fire, summer fire are good ideas, but it needs more
oomph and
DPS for more people to play clerics and do healing.
Maybe make every healing spell that casted by a cleric or favored soul also do divine wrath damage to enemy monster(s) that was hurting the target friend.
If an enemy monster is targeted, the healing spell would in turn do light damage to the target, and healing to the friend who is attacking it.
Also, reactive healing like regeneration is a good idea. So, any dots like Divine Punishment that casts on a monster also provide a healing dot to the friend who is attacking that monster. If casted on a friend, it acts as a healing DOT and provide light DOT to any monster who is attacking that friend.
No, if there's no target selected, then the default target is the caster himself/herself, that will in term protect the caster with regeneration and causes light damage or DOT on monsters attacking the caster.
