2 Rogue/18 wiz (pale master). You get trapping, buffs, dps, self heals (negative energy heals) and evasion (use the feat Insightful Reflexes).

PS: You probably won't need evasion at low levels so take the 2nd level of rogue at level 7 or so and you can back-fill some of your skills.
2nd rogue level at 9 if going PM, lets you get the aura and NEB for self-heals a bit sooner. Plan out the skill point allocation, you can make up a LOT in that one level.

I played like half of my racial lives as a freezy bard with slight variations to accomodate main stat based on race, used harper for int, feydark/purple dragon for char, etc. My favorite split was the 12bard/6fighter/2rog. first lev rog for trap skills, next 3 levs bard for swash, next 6 fighter for kensai core. Then however you want to split levels from there. I always picked up all the cleaves for aoe, full swf, full shield mastery, and ran around in stalwart defender stance for prr/mrr. This allowed plenty of cc with freezes, supercrit profile, full trapping, self heals, dd. I'm sure you could make a DL variant but that'd be focused on t5 dl and not t5wc. keep in mind I just used that as a template/concept and often used harper for trance when another trance didn't favor the races stat bonuses, since int helps with skills/trapping and is a decent second/third stat.
