Often times the game seems to lock up for a few seconds. I dont think it's my internet. Main frustration is it locking up in the middle of combat and dying deep in an instance to have to start over taking xp loss as well. Anything I can do to fix it?

Turn on your network status view in settings. If you're on WiFi, try wired. But odds are you'll see good latency info from the network display and the issues will be entirely on SSG's side; as that's the case for pretty much everyone.

So it normal to get the lightning bar notification on the top of your screen every five minutes or so?

Thats called Lag yes depending on servers

No. This is likely some process somewhere between you and DDO that is cycling. If it's exactly 5 minutes it's definitely a process gone awry.
