Returning noob, have a level 12 sorc I just paid to TR so I can play a build that's relevant. Everytime I try to complete it says name.is already taken or says it can't complete action.... As DDO is filled with bugs, can someone explain this one to me.... Bottom line is I'm WF sorc trying to switch to teifling warlock - why are the error messages not explaining literally ANYTHING.

It's not a TR, it's a lesser reincarnation if you're only lvl 12. If you try picking a warlock pact that conflicts with your alignment then I know it'll give an error stopping you; what's your alignment and what pact are your trying for? (also won't be able to swap race off a LR like that)

The other issue that can happen is if someone's character name is on the updated filter list, but I can't recall what error message it gives for that.

Lesser Reincarnation doesn't let you change your race. So you're not switching from Warforged to anything else before level 20 when you can do a heroic or racial reincarnation.

A +<n> Lesser Heart of Wood will only let you change <n> class levels during the LR. E.g., if you used a +3 LHoW, you could change 3 of your sorcerer levels to something else; if you used a +20 LHoW, you could change all of your sorcerer levels to something else.
