It seems that the optional can't be completed if you first finish the quest. The first time I ran the quest I cleaned the shrine, finished the quest, then collected the herbs (not being sure where to look for them). When I returned to the shrine with the herbs I couldn't finish the optional part.

I reran the quest with the same toon, this time collecting the herbs before finishing the quest and I was able to complete the optional, then finish the quest.

Interesting short quest.

I did it as you did, First completing the quest, then going back a second time, but I never was able to re-dedicate the shrine after obtaining the herbs. I did it with my Paladin who has 18 lvls of Religious Lore. I could always clean the blood, but never re-dedicate. The "shrine always gave me the same options.

There is also a BS lockout on the final fight. I solo'ed the quest and before I started the final fight I went and shrined. When I came back I was locked out by a big red barrier.
