Not sure how I managed to run the entire dungeon, but I killed all the generals, killed the final boss, but somehow failed to complete the objective "explore the troll caves". Thank goodness I was on a character who had the ability to cast Dimension Door so I could go back and figure out what pixel I didn't step on to trigger that particular objective completion. I should have taken note of where exactly that trigger was to report it but I was pretty miffed about the "experience" and time wasted (XP pots per quest completion when?). It ended up being somewhere in one of the side passages with the spiders.

According to the wiki, this should complete when you kill the two trolls at the start of the quest near the locked door - is this not happening?

Yup. I did this once when I was going too fast for my own good, and had to go back to the entrance and walk around there to actually complete it. I forget if I just had to walk around or find and kill the trolls as it was a few years ago, but yeah... that objective completes near the entrance and it is possible to skip it.

Definitely learned that lesson years ago lol. Not sure you have to kill the 2 trolls at the start or just go towards the door.

In Heroic, you have to go to the end of that first hallway to trigger that step. Just killing the first two trolls isn’t enough. Weirdly, I have gotten completions without going as far on Epic.

The ones at the quest entrance (There's a few locked doors in there that players have to pick or bash down)? In any case, I tend to kill as I go so they and all their cohorts were certainly dead behind me.
As for going by the bar door that you open from the other side with a lever, approaching it did not flag that objective. The objective (and quest) ended up completing somewhere in one of the spider tunnels after D-Door run back through the quest to try to find "the spot"

Yes, the quest entrance. Go straight forwards. Where the path splits (forwards to the gate that is opened from the other side, and left to the breakable door to the rest of the quest) there are 2 trolls. Killing them/being in that area is the trigger for that objective. It is possible to go left right away without triggering that objective, so I assume that is what happened here. Simply going back there and not hooking left right away and making sure those 2 specific trolls are dead should resolve the issue.

I am saying "yes, I did that. I double checked that I did when I got to the end and realized the objective didn't complete by D-Dooring back to the quest entrance and running up to it, the proceeded to run through the entire dungeon again until the objective did complete somewhere in a spider tunnel."
