I haven't read all the notes in the Raid yet, but there's an inbuilt story there within the raid itself for anyone interested, which itself is set within a specific location that would permit such scenarios from playing.

Relentless Juggernauts are sourced from the the 5e ravenloft sourcebook with art pretty much matching what we're getting in the raid
They're fiends in humanoid flesh, basically analogous to slasher movie antagonists like Jason and Micheal Myers(not the shrek guy).

Its a bit of a weird case, anatomically a relentless killers are humanoid bodies that have become fiends.
The one in the raid

is a man who gave his body to a fiendish entity in pursuit of vengeance. So in a manner it's a possessed human.

we do see the juggernaut raid boss in the final quest in the saga snowfall and sunlight. unfortunately one of the lore messages is behind a door(the shrine) that never opens so all the lore isn't necessarily to be known as of yet.
