So, uh, does your tank run about wearing the heart on his sleeve all the time or does he make and carry a full heartless swap?

I find the fang of xyzzzy more fun. The set is just a gimmick.

I don't wear it for the set .. or at least I haven't needed to grab artifact PRR that way yet. I wear it for the curse immunity. We do LVOD once a week. It's unbelievably annoying to tank that without a heart.
