Has chest loot changed

I've been getting 1 item (weapon or armor) per chest and then 1-2 gems, 1 scroll, and some gold. I thought chests usually had 2-3 wearable items.

Is this a change or has always been this way?

Always has been.
You get 1 wearable item per chest, with the usual random junk (ammunition, scrolls, gold, gems)
The only time you'll get a second one is if you pull named loot, which is luck-based.
